What Is CountyCare?

CountyCare is a Medicaid managed care plan created especially for residents of Chicago and throughout Cook County. It operates under Cook County Health and is one of the most commonly used options for people who qualify for Illinois Medicaid in this region. CountyCare members have access to a large network of in-network healthcare providers for primary care, behavioral health care, psychiatric treatment, and other mental health and medical services.

CountyCare vs. Illinois Medicaid

Illinois Medicaid is the state’s main health insurance program that helps people with lower incomes get medical care. CountyCare is one of several “health plans” available under Illinois Medicaid, but Cook County Health manages it and mainly serves people who live in Chicago and nearby in Cook County.

If you have CountyCare, you are part of Illinois Medicaid, but you’re using a specific health plan with its own rules and a set list of accepted doctors and clinics (called a provider network). Just because a clinic or psychiatrist accepts “Illinois Medicaid” doesn’t mean they accept CountyCare, so always check whether they are listed as a CountyCare provider before setting up an appointment.

Ketamine Infusions vs. Spravato

Ketamine infusions and Spravato are both treatments derived from the anesthetic ketamine, but they differ in delivery, regulation, and insurance coverage.

IV Ketamine Treatment

Ketamine infusions involve administering ketamine directly into a vein, typically over a series of sessions in a medical setting. These infusions are effective for some individuals with depression or other mental health conditions, but they are considered “off-label” use since ketamine infusions do not have FDA approval for psychiatric indications. As a result, IV ketamine is rarely covered by insurance and is often an out-of-pocket expense for patients.

Spravato

Spravato is an esketamine nasal spray approved by the FDA specifically for treatment-resistant depression. The treatment consists of the patient self-administering the nasal spray under direct medical supervision in a certified healthcare setting. Because Spravato has FDA approval for certain psychiatric diagnoses, it is often eligible for insurance coverage, including policies like CountyCare for qualified patients.

Does CountyCare Cover Spravato Treatment?

Securing CountyCare coverage for Spravato (esketamine) treatment in Chicago involves meeting several medical and insurance requirements. These requirements are typical for Medicaid-managed behavioral health benefits and are designed to ensure that high-level treatments are reserved for patients truly in need and that ongoing mental health care is carefully monitored.

Here are some of the primary factors that determine whether CountyCare will cover Spravato:

Prior Authorization

Before starting Spravato, your healthcare provider, usually an in-network provider, needs to submit documentation and a detailed treatment plan to CountyCare or its partner Medicaid insurance company. The insurance company evaluates whether all criteria are met and will not approve treatment unless they are.

Your mental health care team will need to demonstrate how Spravato fits into your overall plan for managing major depressive disorder or another covered condition, detailing intended follow-up, tapering protocols if needed, and what other treatment options have been considered.

Accepted Psychiatric Diagnosis

CountyCare only approves Spravato for psychiatric conditions supported by clinical research and official plan policy. Generally, this means coverage requires a confirmed diagnosis of treatment-resistant depression or major depressive disorder (MDD) with suicidal ideation. Esketamine treatment for bipolar depression will generally not be covered, because it is considered off-label for that diagnosis. Your prescriber should record how depressive symptoms interfere with your quality of life.

Adequate Medication History

Insurers will ask the treating psychiatrist (the prescriber) to thoroughly document all prior attempts at treatment, usually including a list of traditional antidepressants, psychological therapies, and often inpatient or enhanced outpatient care. They generally want documentation showing that prior treatments did not provide adequate improvement before covering Spravato treatment. Demonstrating a repeated failed medication history will help show why you should be covered.

If everything meets the insurance carrier’s criteria, Spravato treatment could be an option for you under your insurance coverage.
